Assisted Outpatient Treatment is a court-ordered program for treatment of specific individuals with severe and persistent mental illness, who meet strict legal criteria and have a history of poor compliance with treatment. These individuals often experience severe mental health symptoms which impact their ability to recognize the need for treatment. This program is not a crisis intervention program but is intended for the AOT team to partner with clients to achieve and maintain physical and mental health while working to decrease mental health crisis, hospitalizations, incarcerations, and homelessness.
Referrals can be made by an immediate adult family member, an adult living with the client, a licensed professional that is treating the client, a peace office, parole or probation officer, a superior court judge, or a director of a treating agency, organization, facility or hospital. Referral forms can be submitted: by fax at 209-742-0996 or by email firstname.lastname@example.org
